Skilled Nursing
Nursing assessment, treatment and procedures requiring a specialized skill as

ordered by a physician. The nurse will coordinate the home care with the other members of the home care team.

Physical Therapy
Rehabilitation of individuals disabled by strokes, broken bones, trauma or disease. The therapist can help individuals improve their strength, mobility and coordination through exercise, gait training and other treatments.

Occupational Therapy
Provide assistance in relearning or adjusting activities of daily living and in adapting their home for maximum safety and independence.

Speech Therapy
Retraining of individuals who have experienced a stroke, Alzheimer's, tracheotomy, laryngectomy or other disabilities in swallowing and/or speaking.

Medical Social Worker
Provide assistance to individuals to help organize their family's resources and to utilize community resources to solve specific issues.

Home Health Aide
Assist with baths, shampoos and personal care as directed by a nurse or therapist.
